Insurance forms
Specific life events insurance form
Use this form to increase your Death Only or Death and Total and Permanent Disablement (TPD) cover by one unit or 25% of your Fixed benefit cover (up to $200,000) in any 12 month period without having to provide medical evidence because a specific life event has occurred.
Insurance upgrade to Management/Clerical (white collar) scale form
Complete this form to upgrade your insurance cover from the Standard scale to the Management/Clerical (white collar) scale
Insurance transfer form
Use this form to transfer existing Death and/or TPD or Salary Continuance insurance cover from another super fund to HOSTPLUS.
Application to convert from unitised cover to fixed cover
Use this form if you wish to convert your current unitised level of Death Only or Death and TPD cover to a fixed benefit level of cover.
Increase your insurance cover Part A & Part B
